Membership Plans

Empower Wellness Center offers a concierge plan for individuals and families. The plan includes a one-time $10.00 initiation fee and a monthly membership fee. With the membership, patients have access to unlimited primary care (including weight management and hormone therapy) and acute care visits. Functional health visits, such as gut health are not included in this package at this time.

Membership fees:

• Adults: $100 per month
• Children: $30.00 per visit with an active adult parent membership

*Membership plans may be cancelled at any time with a 30-day notice.
**Re-initiation fee: $200. (If you experience a hardship please reach out to us via the patient portal. Your health is important and our goal is to help you stay!)
